Job Description

Comfort Keepers of Fremont is looking for an experienced Lead Caregiver to join our growing family.

The successful applicant will display a compassionate nature, be self-motivated and confident, knowledgeable about health care practices, and provide exceptional customer service. Must be on the California Registry. CNA training a plus.

Please Note: This position has a combination of Field and Office responsibilities.

Field Work: (80%)

  • Interacting with clients directly - daily
  • Providing clients and their families with the care plan of each individual and participate in re-assessments.
  • Routine Care visits
  • In–Home Assessments & Re-Assessments
  • On-call weekend coverage once a month
  • Backup Caregiver for call-outs
  • Coaches caregivers on required skills
  • Assists with local marketing efforts & Health Fairs

Administrative work: (20%)

  • Answering phones & emails in a timely matter
  • Updates CRM system; care plans, daily logs, notes, etc.
  • New referral intake and follow up
  • Proactively schedules/reschedules clients and caregivers
  • Calls to confirm upcoming schedules with client and caregiver
  • Resolves scheduling conflicts in a timely matter
  • Assisting staff with other tasks as needed

Job Type: Flexible Full-time

Job Requirements

  • Work authorization: United States (Required)
  • Must be active on the California Registry
  • Fluent in English, multiple languages is a plus
  • Valid CA Drivers License required
  • CNA, HHA, LVN or Medical Assistant training is a PLUS
