Our home care agency is looking for a Home Care Supervisor with an energetic attitude who is willing to work efficiently and responsibly helping our home care clients and staff. The Home Care Supervisor will be under the direction of the Home Care Managers and the Owners.

The ideal candidate must have a professional demeanor, be a self-starter and a problem solver, be able to think outside of the box, have the ability to multi-task with the ability to handle stressful situations. The Home Care Supervisor must be Team Player and have advanced level experience in Outlook, Microsoft Word, Excel, Window PC’s. Must have advanced knowledge and experience with Hoyer lifts, Medication Administration, HIPAA regulations, Alzheimer's, psychological and physical illnesses. Must also have home care supervisor experience. Case management and bilingual experience a plus.

Our office is located in Aurora, but the position requires travel to clients serviced in Denver Metro and surrounding areas.

 

Job Requirements

Responsibilities/Essential job functions: Employee must be able to perform the following essential job functions -

Demonstrate efficient teamwork with the staff
Home Care Supervisor experience preferred
Must have a valid Driver’s license
Coordinate care for the client. Scheduling.
Understand and perform the admission process plan of care
Supervise field staff 
Provide in-service education per Administrator direction
Attend the initial home care visit and re-evaluate client needs and progress when needed
Coordinate and monitor all client care and services
Complete paperwork timely and efficiently
Maintain client records according to policy and procedure
Follow infection control policy in and out of the office
Provide quality of care to clients in their homes according to policy and procedures
Participate in the on-call schedule
Provide backup caregiving when needed
Be able to answer phone and communicate with office staff after hours
Participate in and support quality improvement programs.
