Job Summary:
The Universal CNA works under the general supervision of both ADHC and Home Care Supervisors.
Responsibilities:
Performs tasks both in the PACE Day Center and in the PACE Participant's home which support the social, physical and cognitive functioning of seniors with impairments.
Demonstrates the knowledge and skills necessary to provide an/or assist with appropriate care to include: personal care (feeding, grooming, bathing, toileting), home services to include laundry, memory stimulation activities (social, recreational), motor activities, escorting to medical appointments, shopping, assisting with transportation of participants to and from the facility utilizing the facility transportation vans and other activities as directed by the Participant's plan of care (POC).
Assists with any identified needs of the Participant during their onsite stay at the PACE facility and in their home that will enable the Participant to perform at the greatest level of independence possible.
