• Valid HHA or PCA certificate
• State issued photo ID Resident card or US Passport Social Security card
• Physical,including height, weight, pulse,blood pressure ( performed within the last 30 days)
• PPD or chest x-ray ( performed within the last 6 months )
• Rubella and Rubeola titers ( performed within the last 5 years)
• Drug Screening ( performed within the last 6 months )
