JOB DESCRIPTION


Position Available: Redford Michigan

 Day Shift Sunday thru Thursday 7am-3pm

Must have valid driver license and clean driving record.

General Statement of Responsibilities:

The Medical Coordinator is primarily responsible for the health, medical care and well being of each person in the home/s he or she is assigned.

 

The Medical Coordinator will promote an unconditional “culture of gentleness” by using positive support and approaches; always treating people with dignity and respect.  He/she will adhere to the Community Opportunity Center, Non-Profit Housing Corporation (COC, NPHC) policies and procedures, public acts, rules and regulations under which homes associated with COC operate including directives and standards set forth by the Detroit Wayne Integrated Health Network (DWIHN).

 

Core Relationships:

ü Report to Home Administrator.

ü Maintain positive and professional relationships with persons served, families, guardians, Power of Attorneys (POA’S), personnel throughout the COC corporation, business partners and members of the community.

 

Job Specific Responsibilities:

ü Guarantee medical, dental and other needs are met.

ü Oversee and document dispensing of medication.

ü Handle medical appointments with follow up documentation.

ü Guide and assist with personal and medical care needs.

ü Work in cooperation with each individual’s physician and specialist to ensure all medical needs are being addressed properly and timely.

ü Monitor staff documentation relative to the dispensing of medication by checking the Medication Book on each day worked.

ü Corroborate all staff is trained and aware of the medical needs of each resident as evidenced by their signature affixed to information presented.

ü Provide in-service training as necessary.

ü Maintain each person’s medical file, including chronological log of required exams, tests, appointments (when each is accomplished), Standard Medical Orders (SMO), Standard Missed Medication Orders (SMMO), prescriptions (including side effects) and recorded weight; all of which is medically necessary.  These are requirements of the Michigan Department of Health and Human

 

Services (MDHHS), Detroit Wayne Integrated Health Network, (DWIHN) and mental health agencies.  All must be documented and maintained in the Medical Coordinator’s Manual.

ü To the extent possible, schedule appointments to ensure the least amount of conflict with other scheduled activities.  Post appointments on a calendar with other scheduled activities that can be reviewed by staff.

ü Accompany persons to medical appointments and hospital visits.  If unable to do so, ensure the staff person who will be escorting the person is knowledgeable about the person’s condition and the necessary follow through as needed.

 

Additional Responsibilities Include but Not Limited to the Following:

ü Verify new medication sheets are in place as updates occur.

ü Guarantee all medications are available, match the medication sheet and prescriptions for each medication.  List all side effect information for each medication.

ü File consultation forms in person’s Medical Book and ensure timely follow up as requested on the form.

ü Confirm all special diet information has been reviewed with staff, posted in the diet book and the diet is properly implemented.

ü Attend to specific responsibilities listed on Medical Coordinator job responsibilities form.

ü Perform other necessary medical measures, ensuring necessary forms and practices are applied.

ü Perform other delegated assignments similar to those assigned to the Direct Care Professional (DCP).

 

JOB REQUIREMENTS


Qualifications:

Must have valid driver license and clean driving record.

1.   High school diploma or equivalent.

2.   Computer skills necessary to maintain adequate electronic records and to provide reports.

3.   Proficient with written and verbal communication skills.

4.   Ability to work cooperatively in a multi-programmed service agency and to work

      independently.  Exhibit emotional maturity and sound judgment.  Handles confidential

      information appropriately.

5.   Proactive approach to problem solving.  Ability to meet deadlines.

6.   Must be at least age 18. 

7.   Have a valid Michigan driver's license and maintain the ability

      to be insured by COC's insurance carrier.

8.   Be able to successfully complete all required Agency training in the required time allotted or

      provide documentation of previously completed training.

9.   Be in good physical health with capability of transferring and/or lifting persons who require

      physical assistance.

10. Be flexible in his/her work schedule.

11. Capable of handling emergencies.

12. No experience required. Willingness to be trained.
